Any experts know what make and model fairing this is, and last but not least if the screens are still available?

If they cannot provide a replacement screen, Gustafsson Plastics can. They made me a screen for a Dunstall fairing and for a Honda CB1100R

I've no idea on the make, EMS is most likely correct..
but if you can't find a screen Zero Gravity still produces many discontinued screens, not sure if they do aftermarket fairings ...it took long wait but they came through with a screen for my '84 GS
that fairing looks very much like the one hanging in my garage , I'll need to bring it down and have a better look when I get back from vacation...
